Sunset Hills Care and Rehab is deeply rooted in the community. Many of the staff are bilingual (speaking both English and Spanish), enabling them to build strong relationships with residents, family members, and volunteers. This has led to a strong intergenerational activities program that involves family members and community volunteers alike. Involvement from family members and individuals throughout the community provides essential social interaction and emotional connection – which are just as necessary to healing and quality of life as are the therapies and treatments we offer. Through the efforts of family and community, and the efforts of our best-in-class staff members, the residents at Sunset Hills are meeting their goals – whether those goals are to regain independence or to receive the medical treatment and care they need in order to enjoy life to its fullest.
At Sunset Hills, therapists employ an individualized and holistic approach to treatment. This begins with the design of a rehabilitation plan – a combined effort by the patient, the Sunset Hills therapy team, and other members of the patient’s care team (such as family members and physician). Such a plan may include physical, occupational and/or speech therapies and is focused on helping a patient regain independence and return to activities previously enjoyed.

The Special Care Unit at Sunset Hills is for individuals with dementia. The unit is designed to keep residents safe. Nurses, with experience in treating those with dementia, staff the unit. The activities program is unique to the unit and is developed to through combined effort by the nursing staff and the Activities Director.
Over 50 million people in the United States are providing in-home, full-time care for elderly loved ones or other family members with disabilities and/or chronic illnesses. Caregivers in such situations need to take time to emotionally and physically recharge. Sunset Hill’s staff will care for your loved one for a short period of time, in our facility, while you take a vacation, deal with other responsibilities, or simply rest and recover.
